Same Day Appointments and Appointment Availability

We pride ourselves in making every effort to accommodate same day appointments. It is very important to us that we are able to provide same day care for injured or sick patients. If you need a same day appointment we encourage you to call early in the day if possible.

**Please call no less than 24 hours in advance of any appointment that you cannot keep, so we can make that time available for other patients. Any cancellations with less than a 24 hour notice will be charged a $40.00 office visit fee or $60.00 for an extended visit or procedure. In addition, these same fee(s) will apply to the second and additional cancellations.

24/7 Coverage

A West Michigan Family Medicine physician is available twenty-four hours a day, 7 days a week, 365 days a year. We encourage you to address routine questions during office hours. We also utilize an answering service for after-hours emergencies. If this service is used excessively or inappropriately a $16.00 charge per call may be incurred.

Your First Appointment

Remember to please arrive early so we can complete your initial paperwork. We also recommend downloading, completing, and bringing in your New Patient Paperwork. In addition to bringing your paperwork, please also remember to bring the following: current insurance card, list of current prescriptions and over-the-counter medication, your medical and surgical history, recent test results and relevant records, preferred pharmacy name, phone number, and location.

Financial Policy

Payment is expected at the time of service unless prior arrangements have been made.  You may pay with cash, check, money order, Visa, Discover, American Express and Mastercard or online through our easy epay form. Health insurance is a contract between you and your carrier. It is important that you understand the coverage that it provides. West Michigan Family Medicine participates with most health plans and will courtesy bill all others.
